Production Records and Meal Service Documentation
Program sponsors that operate the National School Lunch, School Breakfast and/or After School Snack programs, must maintain meal production and service records that demonstrate that the program meals/after school snacks served to students meet minimum meal component and portion requirements for each age/grade group served.
Program sponsors and sponsor vendors should use a standard form to be consistent in documenting program meals and snacks for which a claim for reimbursement has been filed. Completed samples of the Florida Department of Agriculture and Consumer Services’ production record prototype form can be reviewed below.
